Information Collection & Use

Registration

In order to use The Clinic on 24-7 Help, you must first complete the registration form and create a patient name and password. During registration you are required to give contact information (such as name, driver license, permission to contact the Texas Department of Public Safety to verify your driver license, and email address). We use this information to assure that you are a Texas resident, over the age of 21, as well as to be able to contact you about the services on our site in which you have expressed interest.

You must provide demographic information (such as age, address and gender) to us; we need this information so we can make an assessment of your needs as well as an assessment of appropriateness for assistance to you by The Clinic.

24-7 Help will not attempt to contact you except for the following conditions:

  1. You request contact,
  2. The therapist believes it is necessary to contact you.

You may request we contact you by filling out a form on our website. We will respond to you via the Patient Information Page. All communications between patients of 24-7 Help and The Clinic will be through the “patient information page” unless the therapist deems it necessary to use other means to contact you.

24-7 Help is the sole owner of the information collected on www.24-7Help.com. 24-7 Help collects personally identifiable information from our patients at several different points on our Web site.

Services

If you purchase a service from us, we request certain personally identifiable information from you on our order form. You must provide contact information (such as name and email address) and financial information (such as credit card number, expiration date).

We use this information for billing purposes and to provide our services. Service will be provided upon completed credit card processing.

The primary means of communicating with patients of 24-7 Help is the “Patient Information Page”. If the therapist needs to contact a patient through email, patients can expect to receive email communication from the 24-7Help.com domain. If your email address is invalid we may use other means to contact you, such as telephone or mail. However, it is the obligation of the patients to ensure we have a valid email address on record.

Assessments

From time-to-time we may request that you complete Assessments on our site. Participation in these assessments is required for service. During the course of service we may ask you to complete treatment, progress, and termination assessments.

We use this information to determine whether to accept you as a patient, whether to continue to serve you, or whether to terminate service. We also use this information to determine the appropriate service to deliver, and to assess your progress in using our services.

We may use third party service providers to conduct these Assessments; those companies are prohibited from using our patients’ personally identifiable information for any other purpose. We will not share the personally identifiable information you provide through an Assessment with other third parties unless we give you prior notice and choice.

Referral to The Clinic

We do not accept referrals from any source, including satisfied patients. Every patient of The Clinic must submit a request for service directly through 24-7 Help.

Uses of Information

Personal information of any type that 24-7 Help collects is used to assist the visitor in using the web site, to determine whether to offer services, what services to offer, to assess progress, and to determine termination.

Service-related Announcements

We will send you strictly service-related announcements when it is necessary to do so. For instance, if our service is temporarily suspended for maintenance, we might send you an email, or we may post a notice on the Homepage or on your Patient Information Page.

Generally, you may not opt-out of these communications, which are not promotional in nature.

Customer Service

Based upon the personally identifiable information you provide us, we will send you a welcoming email to verify your patient name and password. We will also communicate with you in response to your inquiries, to provide the services you request, and to manage your account. We will communicate with you by email or on your Patient Information Page, as appropriate.

Profile

We store information that we collect through cookies and log files to create a “profile” of your preferences. We share with other third parties only information required to obtain service for, or to deliver service to patients. This information may contain personally identifiable information.

Supplementation of Information

In order to provide clinical services to you, we may on occasion supplement the personal information you submitted to us with information from third party sources.

ID [Address] Verification

We use Texas Department of Public Safety, utilizing the driver license number you provide, to verify your identity and address, in order to assure that we comply with the laws of the state of Texas in providing patient care.

Assessments

We use assessments, whether our own or those from other professional sources to assist in providing clinical services.

Information Sharing and Disclosure

Personally identifiable information:

We DO NOT Share Personally Identifiable Information with Third Parties, except to the Texas Department of Public Safety, in order to assure your identity and age, and to our financial institution, in order to process your credit card.

Legal Disclaimer

We reserve the right to disclose your personally identifiable information as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process served on our Web site.

Log Files

As is true of most Web sites, we gather certain information automatically and store it in log files. This information includes internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages, operating system, and date/time stamp.

We use this information, which does not identify individual patients, to analyze trends, to administer the site, to track patients’ movements around the site and to gather demographic information about our patient base as a whole.

We do link this automatically collected data to your profile. IP addresses are tied to personally identifiable information to track the activities of logged-in patients.

Cookies

A cookie is a small text file that is stored on a patient’s computer for record-keeping purposes. We use cookies on this site. We do link the information we store in cookies to personally identifiable information you submit while on our site.

We use session ID cookies. We use session cookies to make it easier for you to navigate our site. A session ID cookie expires when you close you browser.

We set a session ID cookie to store your passwords, so you don’t have to enter it more than once. Session ID cookies also enable us to track and target the interests of our patients to enhance the experience on our site and to provide access to our services.

If you reject cookies, you may still use our site, but you cannot log in to access The Clinic.

If your personally identifiable information changes, or if you no longer desire our service, you may correct or update it by making the change on your Patient Information Page.

Therapist/Patient Contacts

24-7 Help uses audio/video chat for consultation sessions. 24-7 Help uses the Patient Information Page to communicate with patients. It is the obligation of the patient to check this page regularly for communications from 24-7 Help. Exceptions

Email

Request to reset a password or to request account confirmation process at the time a patient creates an account. In those two events we will use the email address provided.

Other Means

Comply with any legal requirement that the patient be contacted by another means.
